Neurotransmisión glutamatérgica rápida: receptores NMDA y AMPA como moduladores de la excitabilidad neuronal y su implicancia en la excitotoxicidad

Resumen

La sinapsis es el proceso fundamental de comunicación neuronal mediante señales químicas y eléctricas. En la sinapsis química, la más frecuente en el sistema nervioso central, la llegada del potencial de acción despolariza el terminal presináptico, abre canales de Ca²⁺ dependientes de voltaje y activa el complejo SNARE, permitiendo la exocitosis de vesículas con neurotransmisores como el glutamato. El glutamato es el principal neurotransmisor excitador y actúa sobre receptores ionotrópicos NMDA, AMPA y kainato, y receptores metabotrópicos mGluR. Los AMPA generan respuestas rápidas mediadas por Na⁺, mientras que los NMDA requieren despolarización previa para liberar el bloqueo por Mg²⁺ y permitir la entrada de Ca²⁺. La activación coordinada de ambos participa en la plasticidad sináptica, base del aprendizaje y la memoria. En la potenciación a largo plazo (LTP), el Ca²⁺ activa CaMKII y PKC, promoviendo la incorporación de receptores AMPA y fortaleciendo la sinapsis. En la depresión a largo plazo (LTD), disminuye la cantidad de AMPA y se reduce la eficacia sináptica. Sin embargo, la sobreestimulación glutamatérgica puede producir excitotoxicidad. La activación excesiva de NMDA genera una entrada masiva de Ca²⁺, activando proteasas, estrés oxidativo, disfunción mitocondrial y muerte neuronal. Este mecanismo participa en Alzheimer, ELA, Huntington, epilepsia e isquemia cerebral. El equilibrio entre liberación, recaptación mediante EAAT, regulación de receptores y control del Ca²⁺ determina si la señalización glutamatérgica será fisiológica o neurotóxica. Comprender estas vías permite orientar terapias neuroprotectoras dirigidas a modular NMDA, reducir el estrés oxidativo y mejorar la recaptación de glutamato.
