Resumen

La enfermedad pulmonar obstructiva cro´nica (EPOC) es una patologí´a respiratoria progresiva caracterizada por limitacio´n persistente y no completamente reversible del flujo ae´reo, secundaria a una respuesta inflamatoria cro´nica frente a partí´culas nocivas, principalmente el humo del tabaco. Su fisiopatologí´a incluye inflamacio´n cro´nica, remodelado bronquial, aumento de la resistencia de la ví´a ae´rea, destruccio´n del pare´nquima pulmonar con pe´rdida de elasticidad (enfisema) y atrapamiento ae´reo. Estas alteraciones condicionan deterioro del intercambio gaseoso. Asimismo, la EPOC se asocia a manifestaciones siste´micas como disfuncio´n diafragma´tica e hipertensio´n pulmonar secundaria. La comprensio´n integral de estos mecanismos permite optimizar el manejo clí´nico y las estrategias preventivas, especialmente en contextos con acceso limitado a servicios de salud, como el norte del Peru´.
