Introduction: Neonatal dengue is a rare and underdiagnosed form of vertically transmitted infection. Case report: We present the case of twin neonates, one of whom developed clinical signs consistent with dengue infection on day eight of life. The diagnosis was confirmed by NS1 antigen detection. Both the mother and the second twin were asymptomatic but tested positive on rapid tests. Discussion: The clinical presentation was atypical, without fever or jaundice, emphasizing the relevance of differential diagnosis in endemic regions. The outcome was favorable with supportive care. Conclusion: This report highlights the potential for vertical transmission in asymptomatic mothers and the importance of neonatal monitoring during dengue outbreaks.
